We are looking for more patients to join the group and the Chairman would be very happy to speak to you to provide you with more information. So, if you are interested and would like to know more, please speak to the receptionist who will pass your details onto the Practice manager or PPG Chairman will be in contact with you

Travel Vaccinations

Vaccination request form global air travel

If you require any vaccinations relating to foreign travel you will need to complete one of our travel forms in order for our nurses to have a look at to see what vaccinations you may need. Once you have given us your completed travel form please contact reception 3 days later. The receptionist will then make an appointment for you with the practice nurse to discuss your travel arrangements.

There is further information about countries and vaccinations required on the links below

Europe Europe & Russia North America North America
Central America Central America South America South America
Caribbean Caribbean Africa Africa
Middle East Middle East Central Asia Central Asia
East Asia East Asia Australasia Australasia and Pacific

It is important to make this initial appointment as early as possible - at least 6 weeks before you travel. Some vaccines have to be ordered as they are not a stock vaccine, and the vaccines should be given at least 2 weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

If you travel within the next 6 weeks, please consider seeing a local Travel Clinic, as these can have longer opening hours

Travel Health Questionnaire

To help us offer the appropriate advice, please fill out the online form before coming to see the nurse. Please be aware that some vaccinations are not available on the NHS and there will be private fees. Details are on the travel questionnaire.
