Patient Participation Group

The Esher Green Patient Participation Group is now established and will hold regular meetings to discuss matters that are important to you, our patients.

The PPG is a group of patients registered with the surgery who have an interest in the services provided. The aim of the PPG is to represent patients' views and cross barriers, embracing diversity and to work in partnership with the surgery to  improve common understanding.

We are looking for more patients to join the group and the Chairman would be very happy to speak to you to provide you with more information. So, if you are interested and would like to know more, please speak to the receptionist who will pass your details onto the Practice manager or PPG Chairman will be in contact with you

New Patient Registrationregister_now

We welcome new patients who live within our practice area, which is Esher Claygate, the Moleseys and Hinchley Wood. We also partially cover the Dittons and Hersham, but please contact our reception to check these areas.

As we also require ID to register new patients, the registration forms are not available online. To register with our practice please call in to the surgery, ideally between 9.00 and 5.00 when we are fully staffed, to complete the relevant forms, bringing the following ID with you

Photo ID                     This is usually a Passport or Driving Licence

Address Confirmation   A recent utility bill or statement

Children under 16 will be registered, without ID, if parents/guardians are registered here.

Medical treatment is available from the date of registration.

Change of Address

If you move house we need to be informed of your new address in case we need to contact you. You will need to complete a form for each member of your family with your name, old address, new address and telephone number. We will again need a utility bill stating this new address as proof.
