New Patient Registration
We welcome new patients who live within our practice area, which can be seen in the below image. If you are uncertain if you meet our catchment area, please do not hesitate to contact our reception.

To register as a new patient for the surgery please complete the following forms. You need to complete either an adult or child form plus the consent form. You will also be asked to complete a NHS registration form - a GMS1 form which will be provided by the practice.
You must contact the surgery to apply for registration and we will give guidance on completion of application forms. We would appreciate a form of ID and proof of address. If you are applying for on-line access we will need to see identification.
Children under 16 will be registered, without ID, if parents/guardians are registered here.
Medical treatment is available from the date of registration.

Change of Address
If you move house we need to be informed of your new address in case we need to contact you. You will need to complete a form for each member of your family with your name, old address, new address and telephone number. We will need to see proof of your new address. IF YOUR NEW ADDRESS IS OUTSIDE OF THE PRACTICE CATCHMENT AREA - you must register at a practice local to your new address. There is a 28 day period before your records are returned to the PCSE
